Mahoning Valley Nursing and Rehab Center Physical, Occupational and Speech Therapists are health, wellness and rehabilitation professionals dedicated to maximizing function across the lifespan so individuals can live life to its fullest.  Our Therapists utilize a holistic approach that incorporates all facets of wellness to assist residents to age successfully and incorporate wellness beliefs into all aspects of their day-to-day lives.  Our efforts are to empower our residents to take control of their health, well-being, and quality of life while preserving dignity and independence.  The therapists at Mahoning Valley Nursing & Rehabilitation Center work closely with families and caregivers to provide education and training for increased carryover of learned techniques to promote the highest level of independence.

It is important to be aware of the hidden dangers that could be in the home.  In Fact, more accidents happen in the home than anywhere else, especially in the bathroom and the kitchen. As reported by the Centers for Disease Control, 1 out of 3 people aged 65 and older experience a fall every year.  That’s 2.3 million seniors with injuries requiring treatment!  The therapists at Mahoning Valley provide resident and caregiver education for home safety to allow their loved ones to remain in their homes for as long as possible.

 

Some basic recommendations for a safer home environment include:

  • Keeping pathways clear of clutter
  • Removing throw rugs
  • Taping cords to the floor
  • Use of cordless phone/life alert
  • Environmental changes
  • Furniture modifications
  • Use of assistive devices
  • Grab bars
  • Shower/tub chair
  • Reacher/long handle sponge/sock aid
  • Elevated commode

Upon discharge from our facility, safety education for the transition home is discussed on an individual basis according to the resident’s needs.

Our therapists also use a variety of standardized test to assist with implementing the most appropriate treatment plan for our residents.

Tinetti Balance and Gait Test

  • This test is used to measure a residents gait and balance.  It is scored on the resident’s ability to perform specific tasks.

The Times Up and Go Test(TUG)

  • This test is used to assess functional mobility during daily tasks including self care and recreational activities.  The therapist is observing and assessing the resident’s postural stability, safety, gait, stride length, and sway.

The Functional Reach Test

  • This test is used to assess the resident’s stability and safety awareness by measuring the maximum distance an individual can reach forward while standing in a fixed position.  There is also a modified version which requires the resident to sit.

The Montreal Cognitive Assessment (MoCA)

  • This test is used to assess different cognitive domains:
  • attention and concentration, executive functions, memory, language, visuoconstructional skills, conceptual thinking, calculation, and memory.

The Saint Louis University Mental Status (SLUMS)

  • This is another test used to detect cognitive impairment in the areas of orientation, calculations, word finding, visuospatial construction, and memory.

Various Speech/Language assessments are utilized to fully assess skills of residents with speech and language deficits.

The therapists at Mahoning Valley Nursing and Rehab Center are dedicated to helping our residents reach their maximum potential for their individual needs.  Each resident receives an individualized treatment plan on their diagnosis, their personal goals and needs to provide the best possible outcome.

Holidays are a time for friends, family, fun, not to mention lots of food.   Or in the case of Halloween all the wonderful candy and treats. (Like a little chocolate and peanut butter cup maybe? That’s our favorite tell us what’s yours?)

Our Seniors are no exception to enjoying the fun of the holidays.  Halloween is the perfect example of time for a fun holiday that we can share fellowship, games, and some fun costumes.  Oh yeah and maybe a little candy or treats. (We promise not too much though).

Mahoning Valley Nursing and Rehabilitation Center knows how to enjoy Halloween.  We celebrate the holiday with staff and family members dressing up and parade down to our Emerald Grande Room.  Where they parade around for the residents and Judges (which are our residents). To choose the best costumes and have some fun.  The winners do receive a small prize.  The residents also take part in the costume fun by dressing up thanks to staff and family making a fun event for all.  The residents also receive some special treats for Dietary. (Like we said, not too much).

Senior Fair Nov 3, 2017

Join Mahoning Valley Nursing and Rehabilitation Center (MVNRC) Nov 3rd, 2017 for a senior health fair.   We will have some of our friendly staff to discuss services provided by Mahoning Valley Nursing and Rehabilitation Center.  You can have one on one interaction with our staff to discuss topics ranging from aging, health care, and rehabilitation services.  The fair will be held at the Lehighton Recreation Center, 243 S. Eighth St. in Lehighton, Nov 3rd from 10 A.M. to 1 P.M.

Why would you want to attend?  That’s easy there will be a wide array of free health care screening are being offered including cholesterol, blood glucose, blood pressure, EKG, pulse oximetry and hearing. Chair massages (who doesn’t need a massage?) will be offered by The Therapy Option from Jim Thorpe.

Mahoning Valley Nursing and Rehabilitation Center had its 2nd Annual Chopped Competition that took place on Wednesday, September 20th, 2017.

 

The competition is a truly exciting and fun competition where contestants, staff members, compete in a three-round contest. The contestants attempt to incorporate unusual combinations of ingredients into dishes that are later evaluated by a panel of judges. (Sounds exciting right?)

At the beginning of each round (Appetizer, entrée, and dessert), the contestants are each given a basket containing mystery ingredients and are expected to use all of them in some way. The ingredients are often not commonly prepared together. (Let your imagination run wild with you there.) The contestants are given access to a pantry and cooler stocked with a variety of other ingredients utensils to make their masterpieces. Let the fun and food begin.

Did we mention each round has a time limit? (tick, tock, time counts) The time limit may vary from 20 to 30 mins per round. (Sounds like a TV cooking show, right?) The contestants must cook their dishes and complete the plates before the time runs out. (tick, tock,) Once the time has expired, the judges critique the dishes based on presentation, taste, and creativity and select one team to be “Chopped”– Eliminating them from the competition.

Sounds like the folks at Mahoning Valley Nursing and Rehabilitation know how to have a good time, with some good food. The “Chopped” event is a fun example of how Mahoning Valley Nursing and Rehabilitation. It provides exciting events to interact with residents in a game that makes memories and draws residents and staff closer. We hope to see you there next year. Annual Memorial Service

Mahoning Valley Nursing and Rehabilitation Center is holding its Annual Memorial Service

Oct 10th, 2017 at 7:00 P.M.

Every Year we remember and the lives of our residents that passed away in the past year. We hold the service every October and invite the families of these residents. This service is always well attended and features the Schuylkill County Choir and St. Joseph’s bell choir. As we acknowledge each resident, the family is provided with a rose in remembrance. Welcome to Mahoning Valley Nursing and Rehabilitation Center is situated in the foothills of the Pocono Mountains, and only a short drive from the Mahoning Valley exit of the Pennsylvania Turnpike. The facility is compromised of 142 Skilled Nursing beds. Along with our dedicated nursing staff, we have a full complimentary therapy staff who administer physical, speech, and occupational therapy to our residents. We are proud of the fact that many of our residents are admitted for short-term rehabilitation and are able to return to their home or community following their short-term stay. We also offer a comfort care program and hospice services should that need arise.

Located in a peaceful rural setting, the facility is very unique in that it is family owned and operated. At Mahoning Valley, our staff and residents are all part of one family, and we are one of the few skilled nursing facilities to be able to make that claim. It is the hallmark of the very care we give. Others have referred to our care as, “neighbors caring for neighbors”. We are very proud of the care we give and strive to maintain the home-like atmosphere we are known for. Our facility is its own community where care can be given around the clock, every day. Whether you are looking for short-term care, long-term care, or respite care, our staff is able to provide that care 24/7.

We understand the difficult decisions that come with having to place a loved one in a skilled nursing facility. Many members of our staff have, or have had, members of their own family as residents of our facility. We know, from a family member’s perspective, what it is like to be in a situation where long-term care placement is warranted. Our social service department is dedicated to helping families with the admissions process as well as the transition process, and our nurses and certified nurse aides support that transition process as they are rendering direct nursing care.

Our facility has many fine physicians and consultants who provide medical, surgical, and podiatric care to our residents. Care and services, by physicians and physician extenders, is overseen by our facility’s Medical Director. The facility also offers diagnostic laboratory and x-rays services as ordered by the residents’ physicians as well as daily pharmacy services. Our care is a team approach and the staff members of all departments work closely with each of our residents and families to provide individualized care based on that teamwork.

Each of the wings and dining areas of our facility is decorated with hundreds of beautifully framed prints as well as homemade quilts and crocheted items which help to create a warm, homelike, and inviting atmosphere. The facility is centrally cooled and heated, and we maintain a free-standing generator, which is able to fully power the facility, in the event of an emergency, for however long it is needed. These features enable our facility to continue to maintain a homelike and inviting atmosphere no matter what the weather conditions, outside, maybe. A freestanding laundry building provides laundry services for the facility which is also operational every day of the year, in combination with full housekeeping and maintenance services, all of which add another level of comfort, security, and cleanliness for our residents and visitors. Cleanliness and attention to detail is another hallmark of our resident care and services.

We employ a licensed cosmetologist on staff, and our personal care room is equipped to offer hair care services and styling for our residents.

An outdoor porch and courtyard provide opportunities for relaxation and activities for residents and families.
